For twenty years, pharma brand teams measured online presence in clicks and rankings. A patient typed a symptom into Google, scanned ten blue links, and one of them was your brand site. That model is over. Generative AI has already become a primary health information channel at scale, and pharma brand strategy has not caught up.
The scale of the shift: 230 million health questions a week
According to Spectrum Science, ChatGPT alone fields 230 million weekly health questions. That is not a research curiosity. That is a channel larger than most brand teams' combined HCP and patient digital footprints, operating without a single promotional guardrail from the manufacturer. At the same time, IQVIA's March 2026 analysis found that 54% of HCPs now use generative AI in clinical contexts — querying drug information, summarizing trial data, and checking guideline alignment. OpenEvidence, one of the specialist clinical AI platforms, reported 18 million monthly queries from verified physicians. These are not early adopters. This is the median prescriber.
A useful counterweight before overstating the shift: Codeless.io's analysis notes that Google still receives approximately 373 times the search volume of ChatGPT. GEO does not replace SEO — it adds a second, structurally different visibility surface where different rules apply. The brands that treat them as competing priorities will underinvest in both.
The empirical case: AI Overviews are collapsing pharma click rates
The most concrete evidence that generative AI is already affecting pharma brand reach comes from the search results page itself. CMI Media Group's analysis of pharma keyword performance found that AI Overviews now appear on 52% of pharma-related searches. When an AI Overview is present, the organic click-through rate on the same query collapses from 25.8% to 7.4% — a 71% drop. The brand answer is being given before the user reaches a link. If that answer names a competitor, the visit your brand would have received is gone before it began.
This is not a projected risk. It is current performance data from a representative pharma keyword set. For brand teams that fund digital primarily through traffic-based ROI models, the implication is that a meaningful share of organic value is already being captured upstream, in the AI answer layer.
SEO is not dead — but its mechanics have changed
Generative Engine Optimization (GEO) is the practice of ensuring your brand is correctly and completely represented when an LLM synthesizes an answer. It borrows from SEO, but the rules are different at every level:
- The unit changes. SEO ranks pages. GEO ranks claims, citations, and terminology consistency inside one synthesized paragraph. A brand can rank #1 organically and still be absent from the AI answer covering the same query.
- The audience changes. SEO content is read by humans. GEO content is read first by retrieval engines that decide what humans see. Heading structure, claim explicitness, and citation density matter as much as readability.
- The signal set changes. Backlinks still matter as an upstream cause — Ahrefs' 75,000-brand study found a correlation of r = 0.664 between web brand-mentions and AI Overview citation. But the mechanism is different: LLMs use mention consensus across high-authority sources as a proxy for reliability, not just link authority.
- The measurement changes. There is no SERP to count. The metrics are Answer Rate (the share of relevant prompts in which your brand appears at all) and Share of Voice (your brand's share of total named-brand surface area across those answers). Both require structured, multi-engine measurement.
What pharma has that nobody else has — and is wasting
The inputs LLMs trust most are exactly the inputs pharma already produces: peer-reviewed publications, regulatory labels, disease-state guidance from medical societies, patient information leaflets, and real-world evidence registries. No other industry has a comparable body of high-authority, retrieval-eligible content.
The problem is infrastructure, not content volume. Most of this material is locked in PDFs that retrieval engines cannot parse reliably, or scattered across third-party sites that do not link cleanly to the brand. Regulatory labels on accessdata.fda.gov are among the most-cited sources in LLM obesity and metabolic answers — the May 2026 PharmaGEO public index shows FDA labels ranking as the #1 and #2 citation source in the obesity TA — but the labels are produced by regulators, not brand teams. A brand whose own structured content is thinner than its FDA label is ceding the AI answer layer by default.

What we observe in the public PharmaGEO index
The May 2026 PharmaGEO public index measures Answer Rate and Share of Voice for publicly named brands across four therapeutic areas — atopic dermatitis, obesity, psoriasis, and lung cancer — on three engines (OpenAI, Gemini, Perplexity) and three languages (English, French, Spanish). Two findings from this data set illustrate the core problem.
Engine divergence: the 33-point gap on a single brand
In the atopic dermatitis TA, Adbry (lebrikizumab) holds an Answer Rate of 41.4% on OpenAI — a top-4 position in the category. On Perplexity, the same brand on the same week in the same query set scores 8.2% — a rank-8 niche mention. The gap is 33.2 percentage points between two engines measuring the same brand in the same therapeutic area. There is no single "AI visibility" for a pharma brand. There are at least six distinct visibility scores — one per engine-language combination — and they can diverge by a third of the scale.
The implication for brand strategy: any report that quotes a single AI visibility score is measuring one-sixth of the picture. A brand that looks healthy on the engine its team monitors may be structurally absent on the engine its HCPs actually use.
Language flips the leaderboard: the Adtralza phenomenon
In the same atopic dermatitis TA, Adtralza (tralokinumab) holds an English-language Answer Rate of 13.8% on OpenAI — a rank-9 position. Switch the query language to French, and the same brand's AR moves to 48.8% (rank 4). In Spanish: 51.9% (rank 4). A brand that appears to be a minor player in English is a major answer in European-language queries — because tralokinumab received earlier approval and broader clinical coverage in EU markets, and the non-English internet reflects that geography.
The practical implication: a pharma brand's AI visibility is governed by the language of its approval geography, not just its global media investment. A brand that spends 100% of its GEO budget on English content is competing in the wrong pool for much of its European prescriber base.
What changes operationally
GEO is not a new agency line item. It is a rewiring of three existing functions, applied with AI retrieval mechanics in mind:
- Medical Affairs writes for both humans and retrieval engines. Every MOA explainer, clinical summary, and patient FAQ should be published as structured HTML with explicit claim sentences, not as PDFs buried three clicks deep. The front-loading rule from the Princeton GEO study (Aggarwal et al., NeurIPS 2024) applies: 44% of LLM citations come from the first 30% of source content. Lead with the conclusion.
- Brand and Digital measure Answer Rate and Share of Voice in AI answers, not just impressions and organic clicks. A brand can be gaining search rank while losing AI share — the two metrics are moving in opposite directions for many pharma brands in 2026.
- Communications places third-party content with the right attribution language and in the right source archetypes — society guidelines, PMC-indexed literature, specialist hubs — so that retrieval engines have independent consensus to draw on. A single brand site is not sufficient. Consensus across independent, high-authority sources is what retrieval engines treat as reliable.
The compliance dimension no one is planning for
The AI answer layer is not just a marketing surface. It is already a safety information surface, operating independently of the brand team. In the obesity TA, the May 2026 PharmaGEO public index shows that Perplexity cites FDA labels — including boxed warnings, contraindications, and REMS information — as the most-used sources in obesity answers. Brands assuming that AI answers equal marketing copy are already exposed: the answers include mandatory safety information pulled directly from regulatory documents, framed by the engine rather than the brand.
Beyond the safety information already in the label, the index surfaces a more acute compliance risk: AI engines in the obesity TA return mentions of Ozempic and Mounjaro — both approved only for Type 2 diabetes — in response to obesity queries. The surface is measurable but non-zero. Each off-label mention in an AI answer is effectively an unprompted indication expansion, outside any promotional review, reaching prescribers and patients alike. This is a systemic AI behavior, not a brand-specific allegation — but it is a pharmacovigilance and MLR exposure that no brand team's current content strategy is designed to address.
Where to start
The entry point is measurement. Run 80 prompts your target customers actually ask — branded, comparative, disease-state, and safety queries — across at least three engines and two language markets. Score what comes back on visibility, accuracy, sentiment, source quality, citation diversity, and competitive share. That baseline tells you where the gaps are and, critically, which gaps are engine-specific versus universal.
From the audit comes a prioritized 90-day plan. Content infrastructure gaps — unindexed PDFs, missing MOA HTML, absent disease-state pages — close fastest. Syndication gaps take four to six weeks to move citation share. Language gaps require a separate track entirely, because the source pool you are competing in changes with every language.
The brands that establish their AI visibility now will define how their categories are discussed in AI answers for the next decade. The ones that wait will spend those years correcting answers that have become the default — an asymmetric correction problem that gets harder, not easier, with time.
